Health Center Controlled Network
HCP has been a Health Center Controlled Network (HCCN) for over a decade - HCCNs funded by the Health Resources and Services Administration (HRSA), assist groups of health centers working together to address operational and clinical challenges related to the use of health information technology (HIT). HCCNs support community health centers in leveraging health information technology (IT) and data to deliver high-quality, culturally competent, equitable, and comprehensive primary health care. They primarily focus on improvements in clinical quality, patient-centered care, and provider and staff well-being. HCCNs leverage economies of scale such as group purchasing power, shared resources and training, and data analytics expertise to support quality measurement and improvement. This also results in the collective capability to share and apply lessons learned across providers.
The following health centers participate in HCP’s HCCN:
- Community Health Systems, Inc.
- Father Joe’s Villages Family Health Center
