HEALTH QUALITY PARTNERS PROGRAMS
A subsidiary of Health Center Partners, Health Quality Partners (HQP) helps advance health equity and wellness in the communities served by our members. HQP accomplishes this through the implementation of collaborative grant funded programs and research to support community health centers in caring for their patient population. HQP is committed to dissemination of evaluation and research findings to advance both innovative and evidenced-based practices.
EMERGENCY PREPAREDNESS & RESPONSE PROGRAM
Established in 2002, the Emergency Preparedness and Response Program (EPRP) is dedicated to supporting Health Center Partners' member community health centers in San Diego, Riverside, and Imperial Counties to effectively prepare for and respond to public health and all-hazards emergencies. The program's mission is to enhance the resilience and readiness of these healthcare facilities in the face of all-hazards emergencies which include natural disasters, disease outbreaks, and other crises.
By providing direct support and representation at State, Local, Tribal, and Territorial (SLTT) levels, EPRP team plays a crucial role in strengthening the emergency preparedness, response, and recovery capabilities of its member health centers. This comprehensive approach ensures that community health centers are well-equipped to mitigate risks, prepare for potential emergencies, respond effectively when crises occur, allowing them to recover swiftly and continue providing essential healthcare services to their communities.

HEALTH CENTER CONTROLLED NETWORK
HCP has been a Health Center Controlled Network (HCCN) for over a decade – HCCNs, funded by the Health Resources and Services Administration (HRSA), assist groups of health centers working together to address operational and clinical challenges related to the use of health information technology (HIT). HCCNs support community health centers in leveraging health information technology (IT) and data to deliver high-quality, culturally competent, equitable, and comprehensive primary health care. They primarily focus on improvements in clinical quality, patient-centered care , and provider and staff well-being.
HCCNs leverage economies of scale such as group purchasing power, shared resources and training, and data analytics expertise to support quality measurement and improvement. This also results in the collective capability to share and apply lessons learned across providers.

The California Endowment (TCE) Policy and Advocacy Grant
The TCE Policy and Advocacy Grant is designed to advance health policy and advocacy, build an equitable healthcare workforce, and enhance community engagement and mobilization. The program aims to redefine healthcare's role in promoting community health and addressing root causes of disease through ongoing advocacy at various levels, bridging primary care with public health systems, and advocating for health system transformation under CalAIM. It also focuses on creating a culturally responsive workforce, increasing awareness and utilization of allied health workers, and developing career pathways through innovative partnerships. Additionally, the grant seeks to engage health center patients and community members in advocating for health equity and policy changes, ensuring fundamental needs are met and opportunities are accessible for all.
The California Endowment (TCE) Civic Engagement Grant
The TCE Civic Engagement Grant, titled "My Vote. My Health.™," aims to mobilize Community Health Center (CHC) patients, staff, providers, and communities in California to address social and political determinants of health by encouraging voter participation in the 2024 primary and general elections. Running from October 1, 2023, to January 31, 2025, and funded by the California Primary Care Association (CPCA) and The California Endowment (TCE), this initiative seeks to create CHC civic engagement hubs throughout the state. Led by AltaMed and CPCA, the Integrated Civic Engagement Model (ICEM) links healthcare providers with local civic organizations to boost civic participation, aiming to transform underserved communities and improve quality of life. Partners include Health Center Partners of Southern California, San Ysidro Health, Neighborhood Healthcare, Engage San Diego, The Center, and Alliance SD.
