For 40 years Health Center Partners (previously Council of Community Clinics) has proudly served as the primary voice and health care policy advocate for our member community health centers which serve the health needs of communities throughout Southern California in San Diego, Riverside and Imperial Counties. We believe quality health care is a human right.
The Beginning
1977
1977
San Diego County Community Clinics (now Health Center Partners) is established by community clinics in San Diego County.
1978
Developed our first agency goals and objectives "to improve the quantity and quality of health care to the poor"
1979
Formed a Medical and Lab Supply Co-Op (later Council Connections and today CNECT)
1980-1989
1981
Obtained Wholesale Pharmacy Permit which allowed drug orders to be delivered directly to the SDCCC
1982
Executed contract with Scripps to provide services to clinic-referred patients
1983
Expanded Co-Op membership to organizations located in Imperial and Orange Counties
1984
Developed 5-year Strategic Plan with goals including cooperative purchasing, advocacy, technical assistance, shared services, data collection and analysis, and information and referral
1984
Received funding for Hypertension Control Agency (HCA 7) for San Diego and Imperial Counties
1987
Established HIV prevention program
1988
Employed 22 staff and operated 9 programs
1989
Implementation of Tobacco Tax funding (AB 75)
1990-1999
1990
Established the HIV Planning Council in conjunction the San Diego Board of Supervisors
1991
Began the Kaiser Permanente-Community Clinic Partnership
1991
Began long-term contract with County of San Diego to administer the Ryan White Specialty Pool
1993
Established Community Clinics Health Network (today Health Quality Partners of Southern California)
1994
Changed organization name from San Diego Council of Community Clinics to Council of Community Clinics (CCC)
1995
Joined the California Primary Care Association
1996
Completed Medi-Cal health plan contracts for Sharp, CHG, Foundation, Great American, and CompCare
1997
Obtained first out-of-state contract for group purchasing when Planned Parenthood Southwest Oregon joined
1998
Created the Physician Council with clinic medical directors
1999
Began contract with the County of San Diego for telephone triage services
2000-2009
2000
Developed Quality Management Plan to standardize QM activities across clinics
2001
Advocacy activities of clinic consortia statewide coordinated through funding from The California Endowment
2002
Technical Services Organization (TSO) established to provide IT services to clinics
2003
Established community clinic emergency preparedness program
2003
Received federal funding from HRSA for 3-year HCAP diabetes care coordination program
2004
Implemented disease registries in 12 clinics
2005
Received funding from the First 5 Commission of San Diego for a multi-year countywide oral healthcare initiative
2006
Council Connections established as a Premier Contracted Affiliate
2006
Awarded a multi-year contract for Mental Health Services Act (Prop 63) from the County of San Diego
2007
Received federal funding from HRSA for a 3-year project for countywide health information exchange
2007
Council Connections eliminated member service fees

Convened the 1st Annual San Diego Primary Care and Behavioral Health Integration Summit

Celebrated our 40th Anniversary with a celebration at Petco Park
